Disclaimer/Waiver:

 

I believe, to best of my knowledge, the below named student to be in good health, (suffering from no illness and to participate in all types of hockey related training, which demands physical exertion and stamina), and has not been exposed to any infectious disease. It is understood that, except for first-aid treatment, Robb Tallas Goalie Development will accept no responsibility for accident or illness incurred by the student during the program, camp, dryland or any other activity. I hereby give my approval for emergency medical treatment if required.

 

The participants agrees that Robb Tallas Goalie Development , our instructors and the skating rink will not be held responsible for any accidents, injury or loss, however caused and agrees to release the proprietors and/or skating rink from all claims which may arise as a result of any reason of such accidents, injury or loss. I also authorize Robb Tallas Goaltending Development to charge my credit for a non-refundable registration payment for the Goalie named below, with receipt of this application. I am also fully aware of the camp insurance and that all monies paid are non-refundable. Cancellations within 48 hours prior to the camp will be subject to a cancellation fee.
